Your registration date starts the 100-week clock. Everything else is calculated from your log.
Only time after your registration was issued counts. Florida law is explicit: any experience accrued before you registered as an intern does not count toward the requirement (s. 491.0045(4), F.S.). Use your registration issue date, not the date you started working.
Compliance check
Rules from the Florida Board's qualified supervisor guidance. Re-checked every time you add an entry.
Add hours
Log client contact and supervision separately. Supervision entries need a format and whether the session was in person. Starting mid-internship? Enter one large backfill entry for hours you've already accrued, then log week by week from here.

The forms you'll actually need
Straight from the Board, in the order you need them. These link to the official PDFs rather than copies — Board forms get revised, and a stale copy is worse than no copy.
Before you start
- Intern Registration Application DH-MQA 1175 · rev. 5/2025 · $150 fee. Nothing counts until this is issued.
- Qualified Supervisor Affirmation Statement DH-MQA 5049 · your supervisor completes this to confirm they're qualified.
- Mental Health Counseling Education Worksheet Maps your transcript against the required coursework.
- MHC Graduate Practicum Form If part of your practicum was completed outside the academic program.
While you're accruing hours
- Official Supervision Log DH-MQA 5078 · required under Rule 64B4-2.0025. Both you and your supervisor must keep current copies. Electronic signatures are permitted.
- Qualified Supervisor Information Sheet The supervision rules this tracker checks against.
- Address Change · Name Change The Board mails licensure correspondence. Keep these current.
When you apply for licensure
- Application for Licensure by Examination The main application.
- Verification of Clinical Experience DH-MQA 1181 · rev. 5/2025 · this is the form your supervisor signs, certifying 100+ supervision hours over 100+ weeks, 1,500+ direct psychotherapy hours, and supervision at least every two weeks. It's the form this tracker is built around.
- HIV/AIDS Affidavit Required for initial licensure (s. 491.0065, F.S.).
If you're running out of time
- Intern Hardship Form The Board may make a one-time exception to the 5-year expiry in emergency or hardship cases, if you've passed the theory and practice exam. Don't rely on it — but know it exists.
Two rules from the statute that catch people out
Time before registration doesn't count. Under s. 491.0045(4), F.S., any experience completed before your intern registration was issued is disregarded entirely — no exceptions, no backdating.
Private practice requires a licensed professional on the premises. Under s. 491.005(4)(c), F.S., a licensed mental health professional must be on site whenever a registered intern provides clinical services in a private practice setting. Telehealth from home in a private-practice arrangement is where this most often gets missed.

What Florida requires
For registered mental health counselor interns working toward LMHC licensure.
This tool is not legal or board advice. It is a calculator built from the Florida Board of Clinical Social Work, Marriage & Family Therapy and Mental Health Counseling's published supervisor guidance. Rules change and individual circumstances vary. The Board and your qualified supervisor are the authority on your licensure — verify anything here against them before you submit. Check current requirements at floridasmentalhealthprofessions.gov.
Building a practice after licensure?
I'm Zac Dodson — a licensed mental health counselor and qualified supervisor in Orlando. I've helped launch more than 200 practices and I write about the parts nobody teaches you in grad school: credentialing, billing, hiring, and the business of clinical work.
